Undergraduate Pharmacy Intern Per Diem Varied JFK Memorial Hospital POSITION DESCRIPTION: Under the supervision of the Director of Pharmacy or staff Pharmacist, the pharmacist intern is responsible for assuring the drug therapy needs. This individual establishes rapport with other departments as well as the medical staff and nursing services. QUALIFICATIONS: 1. Individual must be a registered pharmacy intern in the State of California and must be awaiting licensure from the California State Board of Pharmacy. 2. Individual shall preferably have a Doctor of Pharmacy degree and have completed a hospital residency. 3. Individual shall be familiar with unit dose, IV admixtures, and clinical pharmacy practice.

Qualifications:
ANDATORY REQUIREMENTS: 1. Requirements: Annual Requirements:  Orientation  Annual Update  Ethics Training  Continuing Education 2. Competency: There are two (2) major areas of responsibility for the intern pharmacist: clinical pharmacy and drug distribution. Clinical Responsibilities:  Interprets and clarifies medication orders and applicable treatments directly from the chart or chart copy.  Maintains patient records to monitor drug therapy for appropriateness and safety consistent with a high level of patient care.  Responsible for assuring that medication schedules are appropriate in relation to the patient’s ability to metabolize and eliminate specific drug compounds (i.e., aminogylcoside kinetcis, geriatric kinetics, heparin dosing, etc.).  Assists the nurse with the routine updating of the patient’s Medication Administration Record (MAR) as appropriate.  Participates in the selection of appropriate medication(s) for specific indications.  Serves as an educator of hospital staff by preparation of clinical conferences and/or “in service”.  Provides discharge counseling to inpatients leaving the hospital with medications when requested.  Participates in drug therapy selection, pharmacokinetic monitoring, culture and sensitivity screening, and drug monitoring/screening of target drug and disease state patients.  Documents interventions/monitoring in a manner consistent with the clinical intervention/monitoring program.  Participates in collection of DUE data on prospective/concurrent basis and intervenes as appropriate when any DUE indicators fall out of the criteria.  Participates in detection and documentation of Adverse Drug Reactions.  Assists in the development of expanding the role of clinical staff pharmacist’s position by actively participating in new and innovative programs Job: Pharmacy Primary Location: Indio, California Hospital Location: John F.
