Social Worker (Unlicensed) - On Call - Telecare Corporation - Indio, CA

The Telecare Riverside County Crisis Stabilization Unit (CSU) is 24-hour, 7 days per week , 12-chair program for individuals experiencing a mental health crisis and who need immediate treatment and support . The CSU will help people find relief from distress, maintain stability, avoid inpatient hospitalization wherever possible, and transition back to their lives with follow-up treatment, resources and community supports in place . This service provides less-than-24-hour care to consumers of all ages . The CSU is housed in the same building as the 16 bed Psychiatric Health Facility (PHF).

As a Social Worker, you will provides psychosocial assessments, group and individual therapy, referrals, and discharge planning to patients. This involves coordinating and collaborating with other services as well as County Mental Health Services

PERFORMANCE STANDARDS

Plans, organizes, directs and controls all functions of the treatment program.

Promotes and maintains harmonious and effective relationships and communications within the program and with allied agencies.

Provides safe, effective and efficient implementation of direct care in accordance with established policies, procedures and standards of care.

Attends and participates in primary therapy, milieu and community meetings, treatment team meetings, and directs groups and individual interventions.

Completes the Psychosocial Assessment for assigned patients

Assumes responsibility for discharge planning, including:
Assessing the patients’ readiness for discharge in coordination with the Treatment Team.

Making appropriate referrals for placement, case management and aftercare services, as assigned.

Coordinates inpatient treatment with referring agencies.

Developing and maintaining an awareness of community resources.

Tracks legal status on all patients, assures that patients know and understand their legal status and rights.

Provides consultation to patients and their families regarding issues arising from the patients’ admission, discharge and/or aftercare plan.

Provide direct treatment for assigned patients.

Observes, records and reports patient social and psychiatric behavior.

REQUIREMENTS

Masters Degree in Social Work from an accredited graduate school; must be license eligible. Two years post graduate experience in a mental health setting required. Necessary skills include the ability to work effectively as a member of a multidisciplinary team, a thorough understanding of patient rights and the conservatorship process, sensitivity to multi-cultural populations and issues, and knowledge of recovery and psychiatric rehabilitation principles. Must have valid CPR certification.
