Job Objective: The Social Worker is responsible for coordinating the complex discharge planning needs of patients and their families, as well as to provide supportive counseling, psychosocial assessment and interventions for patients with complex psychiatric, social, medical and financial needs. Skills Required: Utilizes Windows efficiently for emails and document storage to facilitate intra-hospital communication Utilizes Midas , IKEMD and all pertinent hospital information systems efficiently to document and retrieve patient information in a timely fashion . Education Required: Master’s degree in Social Work Licensure/Certification Required: California Licensed Clinical Social Worker (LCSW) Experience Required: Minimum 2 years clinical social work experience.
